Start with the part that isn’t speculation. In May 2007, Australian customs stopped Stallone at Sydney airport with 48 vials of human growth hormone. He was fined, and his lawyers read a statement to the court admitting the substance was prescribed: “years ago I was diagnosed with a condition and my doctors prescribed human growth hormone and testosterone for its treatment.” He also called the episode “a terrible mistake,” not of intent but of ignorance about Australia’s import rules. That is an official record, not a guess: confirmed HGH and testosterone use, by his own admission, from at least the mid-2000s on.
What that confirmation does not cover is the physique he’s most famous for — the 1980s Rocky and Rambo peak, twenty-plus years earlier. No failed test or admission reaches back that far, so that era gets judged the way every other page on this site is judged: on the timeline.
The timeline
There’s no usable baseline. Stallone says he started lifting at 14, and by the first Rocky (1976) he already had what’s described as a normal, athletic build — but no surviving weight or body-fat number exists for that period. That gap matters: without a “before,” the 1980s jump can’t be measured against his own starting point, only against his later self. Read the verdict on that stretch as correspondingly weaker.
The first hard numbers come from Rocky III (1982), when Stallone reportedly hit 166 lbs (75.3 kg) at what’s widely quoted as 2.8% body fat. We don’t use that 2.8% figure. Single-digit-to-nothing body fat is not a number a human being holds while conscious — competitive bodybuilders at stage-lean, measured by DEXA rather than eyeballed on a film set, usually come in higher than their own guess. We adjust it to a still-extreme 8% for the FFMI math below, and flag the original claim as PR, not physiology.
By Rambo: First Blood Part II and Rocky IV (1985), training twice daily with Franco Columbu, he was reportedly up to 173 lbs (78.5 kg) — no surviving body-fat number, so we carry the same 8% estimate forward, since Stallone himself has said the diet behind that physique was just as extreme as Rocky III’s, down to a stretch of eating nothing but burnt toast.
Run those two points through the site’s FFMI formula at his likelier 5’9” (175 cm) — several independent estimates put him shorter than the 5’10” his official bio claims, and we use the more skeptical number — and Rocky III comes out to an FFMI of about 22.9, Rocky IV about 23.9. The gain between them is roughly 6.5 lbs (2.9 kg) of lean mass over three years, at a training age past twenty years. That’s about 2.2 lbs a year — right at the top of the natural range for a lifter that experienced, not past it. Nothing here needs an explanation beyond a dedicated, decades-in lifter with good genetics and a film budget behind his diet.
Then jump to The Expendables era (2010), when Stallone was reportedly holding a steady 187 lbs (85 kg) at 7–8% body fat for his action films. That’s an FFMI of about 26.0 — over the 25.0 ceiling Kouri et al. found for drug-free lifters — and he was 64 years old. Free testosterone falls roughly 2–3% a year from the mid-thirties on; by every natural expectation, a man’s leanest, most muscular era sits behind him by his sixties, not ahead. Stallone’s numbers say the opposite: his highest FFMI on this page comes at 64, not 39. That is exactly the “improving with age” pattern this site treats as close to decisive — and it lines up, almost too neatly, with an admitted HGH and testosterone regimen that by his own account was ongoing.
What cuts each way
For it: elite, rare genetics; a training age approaching fifty years by 2010; a film industry with no drug testing of any kind, so no institutional check ever applied; enormous financial incentive to keep an action-hero body deep into a career most actors lose by 50. None of that requires drugs to explain the 1980s numbers on their own.
Against it: a physique that got leaner and more muscular from age 39 to 64, against the direction biology runs; an FFMI over the natural ceiling at an age when it should be declining; and, unlike almost every other page on this site, an actual signed admission on the public record rather than a guess from a magazine cover.

Confirmed — but scoped. The HGH and testosterone use is confirmed by Stallone’s own 2007 court statement, and it plausibly explains why his physique held and even improved into his sixties, against what aging alone would predict. The 1980s Rocky and Rambo peak is a separate question with no official record attached to it; on the timeline alone, the numbers we can defend (adjusted for an implausible 2.8% body-fat claim, and missing any pre-1976 baseline) sit inside the range a genetically gifted, decades-trained natural lifter could reach. Two different eras, two different answers — the method doesn’t let one borrow the other’s evidence.
